Creating Forms

You use forms in the EMR. They are linked to a patient's medical history, an episode, or a treatment.

Go to [Settings] > [EMR] > [Forms library].

Click the [+] button to create a new form.

Give the form a name and set the language and specialization. If desired, check the box if the form should have a different heading.
Also set the place of use: the medical history, episode, or treatment. If you want patients to fill out the form, check the box that it should be shareable with the patient.

If you want to keep the content of the form confidential from other colleagues in the practice, check the box that the content is confidential.

Click [Start editing] to open the form editor.

Create the form. Close the form editor when you are finished by clicking the cross.

After closing the form editor, a preview will be displayed. Click [Edit form] to modify the form.
Click [Discard changes] to discard the changes or [Save] to save the form.

After saving, the form will appear in the library. You can edit, hide, copy, or delete the form by clicking the buttons at the end of the relevant row.
